Payment Agreement

The applicant above authorizes L&Y Tax Advisors, LLC to charge my debit/credit card for payment of my invoice(s) from the date of the engagement letter until the expiration of the credit card or termination by the client.

I understand that this authorization will remain in effect until I cancel it in writing, and I agree to notify L&Y Tax Advisors, LLC in writing of any changes in my account information or termination of this authorization. I certify that I am an authorized user of this credit card and will not dispute these transactions with my bank or credit card company; so long as the transactions correspond to the terms indicated in this authorization form.

In the event that this credit card expires or can otherwise not be charged, it is my responsibility as the client to update my card information with L&Y Tax Advisors. If a replacement card is not put on file, a $35.00 fee will be assessed.

I UNDERSTAND THAT BY LEAVING THIS CARD ON FILE, ANY AND ALL INVOICES WITH L&Y TAX ADVISORS, LLC WILL BE AUTOMATICALLY CHARGED 48 HOURS AFTER INVOICING, UNLESS I NOTIFY L&Y TAX ADVISORS LLC OTHERWISE.
